The Partners for Better Care Council (PBCC) is a group of people who have experience in a wide range of health and social care. The PBCC are patients, family members, caregivers and community members. By working together with the West Toronto Ontario Health Team (WTOHT), we want to help make a positive health and social care experience for people living in West Toronto. People who join the Council can help the WTOHT understand the community’s needs and give input on how to help make change.
To help us do this work and to represent the people living in West Toronto, we are asking diverse members from our community to join our Partners for Better Care Council and WTOHT Steering Committee Working Groups. These groups include healthcare providers and patients, caregivers, families, and community members who work together to ensure equitable health and/or social care. Council members will have equal voices in decision making and governance.

The West Toronto Ontario Health Team is working to run workshops, run by community members, for community members to address strategies to support mental health and mindfulness in West Toronto. This program is run in partnership with the Canadian Mental Health Association.
If this is of interest to you, we want to hear from you around how you would like to see these structured, and what topics you would like to have covered. It’s important to us to make sure these trainings are meeting the needs of those accessing them.
These workshops will likely begin in May 2023 and run for a year. If you are interested in registering early or learning more, please enter your email at the end of the survey.

The West Toronto Ontario Health Team (OHT) is currently accepting applications for Partners to sit on the Partners for Better Care Council (PBCC). The Partners for Better Care Council (the Council) is a group of diverse, passionate people with lived experience - who share a desire to improve the experience for patients, families, caregivers, and staff. From their diverse expertise, knowledge and lived experience, partners on PBCC help advance an equitable approach at the OHT through providing key feedback and co-creating engagement efforts for the community.
