West Toronto Primary Care Network

Over the next several months, Department heads of Unity Health will be joining us at our meetings to share information about current services, referral processes and what their key priorities are over the next few years as well as to get input from primary care on how to improve collaboration and communication with family doctors. The Ministry of Health has announced new funding to create more primary care teams across Ontario with a focus on improving primary care attachment rates, especially for those who need it most. The WTOHT will be submitting an application to receive this funding when available and the West Toronto Primary Care Network has an opportunity to provide valuable insight and on the ground experience to aide in the process. 

What are some innovative models that support primary care attachment the WTOHT should consider when applying for this funding? 

This Forum is to start a conversation and gather your thoughts on what should be the most important issues we and our patients face in accessing  health services.

Family Medicine is an amazing speciality because we can design our practice around our skills and areas of interest.  It would be great to come together as family physicians to promote ourselves and our talents amongst each other and our patients.
